Why the form is requested before treatment, what it asks and how each client can share relevant health information privately.
The Health Consultation & Consent Form helps the therapist understand relevant health information, treatment preferences and boundaries before an appointment. It is kept separate from ordinary booking notes so sensitive answers are not placed in general messages.
The aim is not to collect a complete medical history. It is to ask for the information reasonably needed to plan and provide the treatment safely.
The private link is connected to the booking reference, appointment date, time, treatment and intended client. You do not have to re-enter those details or rely on an exact name match for the form to reach the correct booking.
If you prefer a different name on the consultation record, you can update it within the form without breaking that secure booking connection.
The form asks you to confirm that you are 18 or over rather than collecting a full date of birth. It includes focused health and safety questions, preferred pressure and any areas you would like the therapist to focus on or avoid.
Follow-up questions appear only when an answer needs clarification. A ?not sure? option is available where appropriate, and the review step lets you correct an answer before submission.
Your pressure selection is only a starting preference. You can ask for more or less pressure at any point during the appointment. Areas to focus on and avoid are shown in the final summary so they can be checked before the form is sent.
Each person receives a separate private link and answers only for themselves. One client should not complete health declarations for the other person, and neither person can see the other's health answers.
The consultation link expires after successful submission and should not be forwarded. If it expires before completion, Apex can issue a replacement. Sensitive answers are not saved in ordinary browser storage while an unfinished form is being completed.
Tell Apex before treatment if your health, medication, injury or circumstances change. Depending on the information provided, the therapist may adapt, postpone or decline treatment if it cannot be provided safely.
